Basic and more complex forms can be custom built to allow your clients or customers to enter data that will be emailed to you. The data captured by these forms is not stored in the database. You must supply all form design data in electronic format with complete, well-thought-out questions and answers.
Basic Custom Form
A form for clients to enter data into that is emailed to you upon completion. No additional hosting fee required.

A form for clients to enter data into that is emailed to you upon completion. Requires more of our time to lay-up and design appropriate questions and answers. Includes the addition of limited Javascript for creating calendar controls, etc. No additional hosting fee required.
